MEDICATIONS AT SCHOOL

No prescription medication will be given at school unless a “School Medication Prescriber/Parent Authorization Form” has been completed by the parent and prescriber. ALL prescription medication should be delivered to the nurse or medication assistant by a parent or a responsible adult designated by the parent. DO NOT SEND PRESCRIPTION MEDICATION TO SCHOOL WITH YOUR STUDENT.

 

The use of over the counter medication at school is discouraged, unless absolutely necessary. If non-prescription medication is needed more than two weeks, the “School Medication Prescriber/Parent Authorization Form” should  be completed by a  physician and parent.

 

ALL medication must be delivered to the school in the original pharmacy container or manufacturer’s labeled container.
